SHARE-A-SMILE!

This practice provides compassionate, professional care to its patients, and we want our patients to tell family and friends about our services. 

EXAMPLE: 4 NEW PATIENTS = $225 GIFT TO REFERRING PATIENT!

SHARE-A-SMILE!   Refer someone to our office and get a gift certificate as a token of our appreciation.  The referral program has three levels:  Bronze, Silver, and Gold. 

Bronze:  When an existing patient refers a new patient to the practice, the referring patient and the new patient will receive a $25 Bronze gift certificate when the new patient completes his/her first treatment with the practice.

Silver:  If a referring patient has a second new patient to choose our practice; the referring patient will receive a $50 Silver gift certificate and the new patient will receive a $25 gift certificate.

Gold:  Finally, if a referring patient has three or more new patients that choose our practice, the referring patient will receive a $75 Gold gift certificate for each new patient in excess of two and each new patient will receive a $25 gift certificate.

The following restrictions apply to the Share-a-Smile program: (1) gift certificates have no cash value but can be applied toward an account balance or the cost of any non-emergency procedure, (2) a gift certificate cannot be applied toward emergency visits, (3) dental insurance, if applicable, will still be filed but gift certificate can be used to offset any patient responsibility amount remaining after insurance payment, and (4) gift certificates expire one year from issuance, and (5) gift certificates cannot be replaced if lost, stolen, or destroyed.
